There were a number of key findings made by the panel.

"The 9/11 attacks were a shock but should not have come as a surprise," said Republican panel co-chair Thomas Kean. "By September 2001, the executive branch of the U.S. government, the Congress, the news media, and the American public had received clear warning that Islamist terrorists meant to kill Americans in high numbers."

The panel concluded the "most important failure" leading to the Sept. 11 attacks "was one of imagination. We do not believe leaders understood the gravity of the threat."

Among those warnings:

U.S. intelligence agencies failed to appreciate the threat posed by al Qaeda, despite its repeated strikes against American interests worldwide for nearly 10 years before 9/11.

The FBI and CIA, among other U.S. intelligence agencies, failed to adequately share information between bureaus.

U.S. intelligence employed poor analysis of available information, leading to "colossal system failure."

Congress failed to employ adequate intelligence oversight.

The commission found evidence of ties between al Qaeda and "axis of evil" state Iran.

The panel identified nine "specific points of vulnerability" that could have led to the disruption of the attacks had the government been more vigilant and more observant of intelligence signs.

Iraq and al Qaeda had explored an alliance in the early 1990s, though a formal agreement never came to fruition.Find out more about the 9/11 Commission's report, their revelations, and their blueprint for how we can win the war on terror -- Go Here Now.
